This is the data collection exercise for next academic year

 The information we hold is:

  2. Chosen Name of your child/ren
  3. DoB
  4. Gender
  5. Address/post code
  6. Telephone numbers
  7. Email address
  8. Ethnicity
  9. Religion
  10. Country of birth
  11. Nationality
  12. Home language
  13. First language
  14. Dietary needs
  15. Medical conditions
  16. Disabilities
  17. Medical Practitioner’s address and telephone number
  18. Names, addresses, contact details of people who we will call in an emergency.

 

 If there are no changes to the information since last year, then please do not complete the form.

 If there are changes, please click on the link below and complete the Google Form.

 

Please note: we MUST have more than one emergency contact listed. We have allowed for 4 on the form, but you only have to complete for 2 before submitting the form. If you know that we only have 1 contact on our system but all other information is correct, then please email into the office the additional information and we will add to your details.
